Instructions

This form was designed to facilitate communication between the MONICA Data Centre (MDC) and the MONICA Collaborating Centre (MCC) about possible errors or uncertainties for data previously reported by the MCC to the MDC on a magnetic tape. It is initiated by the MDC, which completes parts A, B, C, D, and the other columns except "CORRECT VALUE" of the list of constraint violations and sends the otherwise blank form to the MCC. The MCC should then complete the Error Correction Form and return it to the MDC. The MCC should keep a copy of the completed Error Correction Form.

For errors detected by the MCC but not detected by the MDC, Form G: Manual Error Correction Form for Data Transferred on a Magnetic Tape should be used.

This form is sent to the MCC, with items A, B, C, D, completed with a list of constraint violations, soon after the MDC receives the data from the MCC. It is used only if there are questions about the information provided. The remainder of the form is to be completed at the MCC by a person designated by the Principal Investigator to perform this task.

General instructions for completing form

This form and the parts to be completed by the MDC are computer generated. Starting from page 2, the format has a common heading as follows:

The data in the heading corresponds to items A, B,... of the first page of the form. Also, the heading indicates the page number.

This form can be completed either by typewriter or by hand. In either case the recorded information should be examined carefully for both correctness and legibility. Further, since it may be necessary to photocopy the form, perhaps several times, as a routine part of processing and providing reserve copies, it is essential that the recorded information should stand out clearly. Hence, please use a dark ribbon if a typewriter is used, or a dark ball-point pen if the form is completed by hand.

This form includes blank spaces where you are to record the requested information. None of these spaces should be left blank, except when a value is correct, when the first box only is ticked. If you have no information to provide for a specific item, then so indicate according to the instructions for that item.

The first 5 columns of this page are filled by the MDC. One line is reserved for each possible inconsistency noted by the MDC.

Column "Serial number"
indicates the serial numbers of the observations with possible inconsistencies.

Column "Constraint name"
indicates the name of the MDC database constraint which is being violated. The constraints have been described in the "Edit Specifications" which have been distributed to the MCCs separately.

Column "Constraint status"
is described on the first page of the computer generated error correction form.

Column "Item name"
indicates the name of the data item of the possibly inconsistent value, as given in the data transfer form identified in item A of this form.

Column "Present value"
indicates the value being queried by the MDC which is the value that it presently has in its files.

The column "Correct value"
is to be completed by the MCC. The MCC should check all the possible inconsistencies reported on this form against the original documents in which the data were recorded.

If the value, as written in column "Present value" of this form, is correct, a tick ("v") should be coded in the first column of the "Correct value".

If the value of "Present value" is incorrect the correct value should be coded in "Correct value".

Queried values which cannot be shown to be erroneous should not be changed. Instead, such values, even though very unusual, should be confirmed.

No space in the form should be left blank, except when a value is correct when the first box only is ticked.

This form should only be used to correct errors identified by the MDC and printed on this form. If other errors are discovered they should be corrected using Form G: Manual Error Correction Form for Data Transferred on a Magnetic Tape.

Correcting mistakes: If a mistake is made in entering information for this item, please draw a single line through the erroneous entry and provide the correct information immediately to the right. Even if only one digit of the code number is incorrect, please draw a line through the entire number and re-write the correct number immediately to the right as indicated in the completed example of the form following these instructions.
