The MONICA Data Centre (MDC) has checked the data identified above for
possible double registrations. All situations where two event records had
the same sex, the same date of birth and the dates of onset were within the
same 28-day period are listed on this form. It is possible that both events
occurred to two different persons and therefore are valid. However, if the
records refer to the same person, then the same event has been registered
twice, and one of the records should be deleted from the MONICA Database.
Please review carefully every record whose serial number is reported below
and code in the box the result of your review using the following codes:
1 = The record corresponds to a separate event, and
is correct.
2 = The record corresponds to a separate event, but
has error(s) in date of birth, date of onset or sex.
A MANUAL ERROR CORRECTION FORM (Form G) for the event
record is enclosed.
3 = The record is a double registration of an event,
and therefore should be removed from MONICA Database.
Every box of this form should be completed with either code 1 or 2 or 3.
