Instructions

The purpose of this form is to provide a common format for the labels to be fixed on magnetic tape reels or on diskettes.

A label has to be completed every time data are submitted in a magnetic tape or diskette from the MONICA Collaborating Centre (MCC) to the MONICA Data Centre (MDC). If several tapes or diskettes are sent at the same time separate labels have to be completed for each tape or diskette.

The label should be completed according to the instructions below. It should not be completed on this form but on a sticker which is identical to the part below the heading of this form. The sticker should then be fixed on the reel of the magnetic tape or directly on the diskette.

The sticker has to be completed and fixed on the reel by the person who is responsible for writing the data on the tape or diskette in the MCC.

Distribution: The MDC prints stickers. The MCC is responsible to request more stickers from the MDC when needed.

General instructions for completing label

The label can be completed either by typewriter or by hand. In either case the recorded information should be examined carefully for both correctness and legibility. It is essential that the recorded information be clear and dark. Hence, please use dark ribbon if typewriter is used or record in black ink.

The label includes blank spaces where you are to record the requested information. None of these spaces should be left blank.

If you make a misprint in the label please discard the incorrectly completed label and complete another label and fix it on the reel.

Specific instructions for each item

These instructions should be followed carefully when completing the indicated version of the TAPE LABEL. Please ensure that the instructions are for the version of the label being completed. Specific instructions are listed by item below:

Print or type the official MONICA Collaborating Centre code number in the space provided. Use the code number as it appears in Part I, Appendix 2: MONICA Collaborating Centres and Reporting Units.

Print here the date when the tape or diskette was recorded. The date should be the same as in the TAPE HEADER. Give the date in the form: day/month/year.

Print the TAPE NUMBER in the space provided. The number must have four digits. (If necessary print 0's in front of the number.) The number must be the same as the Sequential Tape Number recorded in the TAPE HEADER and TAPE TRAILER.

Print or type in the space provided the density on which the tape was written. Use units Bytes per inch. Note that MDC accepts tapes written on densities 800, 1600 and 6250. If you send data on diskette, please draw a horizontal bar in the space. In such case the item is irrelevant.

Type or print here the record length used. If you send data on diskette, please draw a horizontal bar in the space. In such case the item is irrelevant.

Print or type the character code used. Note that the accepted codes are ASCII and EBCDIC. Note that only ASCII is accepted if you submit data on a diskette.

If the tape is unblocked print here "U.B.". Otherwise print the block size in bytes. If you send data on a diskette, draw a horizontal line in the space. In such case this item is irrelevant.
