The purpose of this form is to provide an exact and common format for the tape header (i.e. for the first record) of a magnetic tape or diskette in which data are transferred from the MONICA Collaborating Centre (MCC) to the MONICA Data Centre (MDC). The automatic data processing routine of the MDC uses the information of the TAPE HEADER to find out what kind of data to expect from the tape or diskette and to check that the data found corresponds to the data expected. Thus it is important that the TAPE HEADER is recorded correctly.
The ITEM NAME on the form is a computer variable name used for the item by the MDC. The COLUMNS indicate the columns on which the value of the item is to appear in the record on the magnetic tape or diskette.
None of the characters of the form should be left blank.
These instructions should be followed carefully when the TAPE HEADER is created. Please ensure that the instructions are for the version of the TAPE HEADER form being used. Specific instructions are listed by item below:

Enter "HDMONICA" to indicate the TAPE HEADER.

Enter the version number printed on the TAPE HEADER form. If the number is not 2 these instructions do not correspond to the form version you are using. Check that you are using the valid version of the form.
HDCENTRE MONICA Collaborating Centre code
Enter the official MONICA Collaborating Centre code number as it appears in Part I, Appendix 2: MONICA Collaborating Centres and Reporting Units.

Enter here the TAPE NUMBER of the magnetic tape or diskette. Instructions for creating the number are given in section 3.1.3.1.

Enter the date when the tape is written such that the date is coded in the first two columns, the month in the next two columns and the last two digits of the year in the last two columns. For days and months 1 to 9 code 01 to 09. The date should be the same as the date written on the TAPE LABEL.

Give here the total number of files in the tape or diskette which contain forms with form identification 1, 2,..., 8 or 9. The number given here should be the sum of NUMFILE1 to NUMFILE9.

Indicate here the number of files which contain forms "CORE DATA TRANSFER FORMAT - CORONARY EVENTS". By definition, such files are sets of records starting with a FORM HEADER and ending with the corresponding FORM TRAILER. Note that data from different Reporting Units must be submitted to the MDC on different files. If no files containing form 1 are sent, 00 should be entered. For numbers 0 to 9 code 00 to 09.
This item is used in the MDC to check that every file contained in the tape has been read in the MDC.
