The purpose of this form is to provide an exact an common format for the form header, which is the first record of a file containing actual MONICA data in the magnetic tape or diskette in which data are transferred from the MONICA Collaborating Centre (MCC) to the MONICA Data Centre (MDC). The automatic data processing routine of the MDC uses the information of the FORM HEADER to find out what kind of data to expect in the file and to check that the data found corresponds to the data expected. Thus it is important that the FORM HEADER is recorded correctly.
The ITEM NAME on the form is a computer variable name used for the item by the MDC. The COLUMNS indicate the columns on which the value of the item is to appear in the record on the magnetic tape or diskette.
None of the characters of the form should be left blank.
These instructions should be followed carefully when the FORM HEADER is created. Please ensure that the instructions are for the version of the FORM HEADER format being used. Specific instructions are listed by item below:

Enter "HDRFORMS" to indicate the FORM HEADER.

Enter the version number printed on the FORM HEADER form. If the number is not 3 these instructions do not correspond to the form version you are using. Check that you are using the valid version of the form.

Enter here the form identification of the data forms for which actual MONICA data is included in this file. The accepted values here are 01, 02,..., where 01 means the CORE DATA TRANSFER FORMAT - CORONARY EVENTS etc.

Every form (excluding the FORM HEADER and FORM TRAILER) of a Data File must have the same number of records per form.
